Whether you live in the snow belt or the sunny south, the winter season often means a change in the weather. In many places, the sun isn't as strong and cooler temperatures bring relief from the summer's heat. Yet even though it may be chillier outside, your body's need for hydration is the same as it was in the summer—and a lack of proper hydration can be bad news for your oral hygiene.

Everyone knows we need to drink plenty of water every day to stay healthy. It's important for good oral hygiene because water is the major component of saliva, which fights bacteria and helps neutralize the acids that cause tooth decay. Water also keeps the soft tissues of the mouth moist and healthy, and helps fight bad breath. In many communities tap water is fluoridated, which offers proven protection against cavities.

But in the middle of winter, fewer people carry around bottles of cold water for refreshment—and that's a shame, because we need it just as much! While indoor (and outdoor) air is often drier in winter, your body continues to lose water in the same ways. And if you keep up a healthy exercise routine (like jogging, snow sports or backyard fun and games), you still need plenty of hydration. An ice-cold glass of water may not be as appealing in January as in July…but it's just as important.

Of course, the water you drink doesn't have to be freezing cold to do its job. Hot tea (especially herbal tea) can be a healthy option for wintertime hydration. So is plain water without ice. Fruits and vegetables also contain lots of water, plus vitamins, fiber, and many more substances that are good for your body.

But there are some drinks you should avoid—or at least take in moderation. Regularly drinking coffee and tea can stain your teeth, and excessive caffeine may have negative health effects. Consuming alcoholic beverages can cause dry mouth, and may increase the risk of oral cancers. And, of course, drinks that contain sugar (including soda, some juices, and many coffee and hot chocolate beverages) are linked not only to tooth decay, but to other health problems as well.

And whatever the season, don't forget to come in to the dental office for regular checkups and cleanings. We can remove the sticky tartar that clings to your teeth and may cause tooth decay and other problems. Around 20 million people—mostly women after menopause—take medication to slow the progress of osteoporosis, a debilitating disease that weakens bones. But although effective, some osteoporosis drugs could pose dental issues related to the jawbones.

Osteoporosis causes the natural spaces that lie between the mineral content of bone to grow larger over time. This makes the bone weaker and unable to withstand forces it once could, which significantly increases the risk of fracture. A number of drugs have been developed over time that stop or slow this disease process.

Two of the most prominent osteoporosis drugs are alendronate, known also by its trade name Fosamax™, and denosumab or Prolia™. While originating from different drug families, alendronate and denosumab work in a similar way by destroying specialized bone cells called osteoclasts that break down worn out bone and help dissolve it. By reducing the number of these cells, more of the older bone that would have been phased out lasts longer.

In actuality this only offers a short-term benefit in controlling osteoporosis. The older bone isn’t renewed but only preserved, and will eventually become fragile and more prone to fracture. After several years the tide turns negatively for the bone’s overall health. It’s also possible, although rare, that the bone simply dies in a condition called osteonecrosis.

The jawbones are especially susceptible to osteonecrosis. Forces generated by chewing normally help stimulate jawbone growth, but the medications in question can inhibit that stimulus. As a result the jawbone can diminish and weaken, making eventual tooth loss a real possibility.

Osteonecrosis is most often triggered by trauma or invasive dental procedures like tooth extractions or oral surgery. For this reason if you’re taking either alendronate and denosumab and are about to undergo a dental procedure other than routine cleaning, filling or crown-work, you should speak to your physician about suspending your medication temporarily. Dentists often recommend a suspension of three to nine months before the procedure and three months afterward.

Some research indicates this won’t worsen your osteoporosis symptoms, especially if you substitute another treatment or fortify your skeletal system with calcium and vitamin D supplements. But taking this temporary measure could help protect your teeth in the long run.

While they're resilient, your child's teeth aren't invincible. Daily hygiene and regular dental visits are important, but you should also be alert for problems and take action when they arise.

Here are 4 areas that could cause problems for your child's teeth, and what you should do — or not do — if you encounter them.

Teething. This is a normal experience as your child's first teeth erupt through the gums. The gums become tender and painful, causing constant gnawing, drooling, disturbed sleep and similar symptoms. You can help relieve discomfort by letting them bite on a chilled (not frozen) teething ring or a cold, wet washcloth. Pain relievers like ibuprofen in appropriate dosages can also help — but don't apply ice, alcohol or numbing agents containing Benzocaine directly to the gums.

Toothache. Tooth pain could be a sign of decay, so you should see us for an examination. In the meantime you can help relieve pain with a warm-water rinse, a cold compress to the outside of the face, or appropriately-dosed pain relievers. If the pain is intense or persists overnight, see us no later than the next day if possible.

Swollen or bleeding gums. If you notice your child's gums are red and swollen or easily bleed during brushing, they could have periodontal (gum) disease. This is an infection caused by bacterial plaque, a thin film of food particles that build up on the teeth. You can stop plaque buildup by helping them practice effective, daily brushing and flossing. If they're showing symptoms, though, see us for an exam. In the meantime, be sure they continue to gently brush their teeth, even if their gums are irritated.

Chipped, cracked or knocked out tooth. If your child's teeth are injured, you should see us immediately. If part of the tooth has broken off, try to retrieve the broken pieces and bring them with you. If it's a permanent tooth that was knocked out, pick it up by the crown (not the root), rinse it with clean water and attempt to place it back in the socket. If you can't, bring the tooth with you in a container with clean water or milk. The sooner you see us, the better the chances for saving the tooth — minutes count.

There’s a potential threat lurking in your young child’s mouth—tooth decay. This destructive disease can not only rob them of teeth now, it could also impact their dental health long into their adult years.

That’s why we focus heavily on decay prevention measures even in primary (“baby”) teeth, as well as early treatment should it still occur. It’s a straightforward treatment strategy: minimize the factors that contribute to disease and maximize those that protect against it.

We can represent the disease-causing factors with the acronym BAD. Bad bacteria top the list: they produce oral acid that erodes tooth enamel. Couple that with an Absence of healthy saliva function, necessary for acid neutralization, and you have the potential opening for tooth decay. Poor Dietary habits that include too much added sugar (a prime food source for bacteria) and acidic foods help fuel the decay process.

But there are also SAFE factors that can help counteract the BAD. Promoting better Saliva function helps control acid levels, while Sealants applied to chewing surfaces strengthen these vulnerable areas against decay. We can prescribe Antimicrobials in the form of mouth rinses that reduce abnormally high bacterial concentrations. Fluoride applied directly to the enamel bolsters its mineral content. And an Effective diet high in nutrition and low in sugar or acidic foods rounds out our protective measures.

Promoting SAFE factors greatly reduces the risk of childhood tooth decay. To keep on track it’s important to start regular, six-month dental visits beginning around your child’s first birthday. These visits are the most important way to take advantage of prevention measures like sealants or topical fluoride, as well as keeping an eye out for any signs of decay.

And what you do at home is just as important. Besides providing a teeth-friendly diet, you should also brush and floss your child’s teeth every day, teaching them to do it for themselves when they’re old enough. Playing it “SAFE” with your child’s dental health will help ensure your child’s teeth stay decay-free.

The vast majority of teeth and gum problems stem from two dental diseases: dental caries (tooth decay) and periodontal (gum) disease. But although these dental diseases are all too common in our society, there’s a good chance you can prevent them from harming your own dental health.

That’s because we know the primary cause for both of them—dental plaque, a thin film of bacteria and food particles that can build up on tooth surfaces usually as a result of poor oral hygiene. Remove this plaque build-up daily and you dramatically decrease your risk for disease.

The primary way to do this is with a daily habit of brushing and flossing. While regular dental cleanings remove plaque and tartar (calcified plaque) from hard to reach places, it’s your regular practice that removes the bulk of daily buildup. Interrupting plaque buildup helps keep disease-causing bacteria at bay.

That also means performing these two hygiene tasks thoroughly. For example, you should brush all tooth surfaces, especially in the rear and along the entire gum line (a complete brushing should take at least 2 minutes). And by the way, “thorough” doesn’t mean “aggressive”—a gentle circular motion is all you need. If you scrub too hard, you run the risk over time of damaging your gums.

And while many people discount flossing as a hard and unpleasant task, it’s still necessary: at least half of the plaque in your mouth accumulates between the teeth where brushing can’t reach effectively. If you find flossing too difficult, you can take advantage of tools to make the task easier. A floss threader will make it easier to get floss through your teeth; you could also use an oral irrigator, a device that emits a pressurized spray of water to loosen and flush away some plaque.

Along with dental visits at least twice a year, daily brushing and flossing is the best way to reduce your risk of both tooth decay and gum disease. Avoiding these two diseases will help ensure your smile is attractive and healthy throughout your life.
